5 SCREENINGS+1 LIFESTYLE CHANGE=20 YEARS OF LIFE (5+1=20)
This program is designed to bring awareness to the risk factors and frequency of the top five chronic diseases that affect certain communities in the Chicagoland area as well people across the nation.
The purpose is to increased prevention of these illnesses through education and pre- screening rather than just treatment of diabetes, cancer, asthma, heart disease, and HIV/AIDS.
Students will learn about common chronic diseases and understand how these diseases can be prevented and managed. In addition, they invite participants to attend their health fairs hosted twice a year and encourage each participating student to bring at least 10 family/community members to engage in education and pre-screening of some chronic diseases.
PILSEN NEIGHBORS COMMUNITY COUNCIL (PNCC)
The Pilsen Neighbors Community Council (PNCC) is a community-based organization that benefits the community by developing leadership training to bring about civic engagement to the ward and help educators,
the community and government to improve the health and welfare of the overall community.
EIGHTEENTH STREET DEVELOPMENT CORPORATION (ESDC)
Eighteenth Street Development Corporation (ESDC) envisions Pilsen and the Lower West Side of Chicago as a vibrant center of sustainable commercial and industrial activity, offering safe modern housing, and a diversity of employment opportunities.
The ESDC has served the Pilsen community on the Lower West Side of Chicago in Cook County, Illinois for over 38 years and looks forward to adding to its legacy.
The ESDC will work with businesses to empower them to grow. It will also work to attract fast growing businesses by leveraging the community’s assets: an ideal central location,
rich in culture and arts, delicious food/restaurants, a close proximity to world class universities and medical district, in a Hubzone, with resources like TIF, and Federal Empowerment programs.
The Resurrection Project is a community based organization that challenges individuals to act upon their faith and values.
They have provide many programs that empower the community such as immigration services, education,
financial wellness, safety, health, and home purchase programs.
MUJERES LATINAS EN ACCION
Mujeres Latinas en Acción is an organization that empowers Latinas through service.The agency serves as a guide to both Latinas and their families.
The Mujeres Latinas en Accion provides multiple programs and services with includes: after school programs for youth and teens, parent support programs, counseling, crisis intervention,
and court advocacy for survivors of domestic violence and sexual assault, court supervised visitation for noncustodial parents and referrals to medical and financial support resources for women.
El Valor is a community-based organization that strives to build a community in which all of the members,
including individuals with disabilities, can live, learn and work independently and with the community.
El Valor aims to enrich and empower all those with disabilities and their families.
CHINESE AMERICAN SERVICE LEAGUE (CASL)
The Chinese American Service League (CASL) is a social service agency that serves the needs of Chinese Americans.
They focus on making all ASL clients independent and productive members of society by providing strengthening cultural heritage,
providing an educational and workforce foundation and securing community housing.
